Evaluating the appropriateness of antimicrobial treatment in hospitalized patients: a comparison of three methods

背景
入院患者における抗菌薬療法の約20％ ～ 50％は不適切と考えられ、これは合併症および死亡の増加と関連する可能性がある。抗菌薬療法の適切性を評価するための最良の方法は確立されていない。
目的
2 次病院における適切な抗菌薬療法の実施率を、3 種類の方法を用いて評価すること、および異なる方法間の一致度を明らかにすること。
方法
2016 年中に全身抗菌薬療法を受けたすべての成人入院患者を対象として、単一の日にスクリーニングを行い、点有病率研究を実施した。患者ファイルから臨床データ、検査データおよび治療データを収集し、専門家の意見に基づいた定性的評価により適切性を判定した。さらに、各患者について判定した 11 の質的指標に従って定量的評価を実施した。適切性の厳格な定義は 6 つの重要な質的指標が満たされることとし、あまい定義は少なくとも 5 つの質的指標が満たされることとした。各方法間の一致度はκ統計量を用いて分析した。
結果
組み入れた患者 106 例中、抗菌薬療法が適切であった割合は、評価方法により 20％ ～75％の範囲であった。厳格な定義と専門家の意見との間の一致度は非常に低く（κ ＝ 0.068）、あまい定義と専門家の意見との間の一致度は中等度であった（κ ＝ 0.45）。
結論
抗菌薬療法が適切であった割合には評価法によってばらつきがあり、異なる方法間の一致度は低度から中等度であった。

抗菌薬の適正さを確実に評価できる方法は臨床分離菌とその疫学的背景を照合したり、臨床分離菌を確保し具体的な感受性情報を入手できるからである。感染症のすべての場合に、臨床分離菌が特定できるわけではないので、その点に注意が必要である。
