Rapid emergence of colistin resistance and its impact on fatality among healthcare-associated infections

本稿では、2014 年から 2015 年に発生した医療関連グラム陰性菌血流感染症 1,556 症例の耐性出現と死亡の予測因子について報告する。肺炎桿菌（Klebsiella pneumoniae）のコリスチン耐性率は、2013 年では 6％であったのに対し、本研究では 16.1％であった。計 660 症例（42.4％）が死亡に至った。死亡率は、アシネトバクター・バウマニー（Acinetobacter baumannii）（58％）菌血症の患者で最も高く、続いて、緑濃菌（Pseudomonas aeruginosa）（45％）、肺炎桿菌（41％）、エンテロバクター・クロアカエ（Enterobacter cloacae）（32％）、大腸菌（Escherichia coli）（28％）菌血症の患者で高かった。多変量解析により、カルバペネム［オッズ比（OR）1.02、95％信頼区間（CI）1.01 ～ 1.04、P ＝ 0.002］およびコリスチン（OR 1.1、95％CI 1.03 ～ 1.17、P ＝ 0.001）の最小発育阻止濃度は、死亡と有意に関連していることが示された。
