腎盂腎炎の疑いと微生物学的診断との不一致：英国の 1 教育病院におけるコホート研究

尿路感染症は、救急部におけるエンピリック（経験的）な抗菌薬投与の理由として多くみられる。本研究では、ロンドンの教育病院 1 施設において腎盂腎炎の臨床診断を受けた患者 105 例を対象に、腎盂腎炎の診断における培養検査および尿検査の役割を検討した。全体で、102 例中 99 例が抗菌薬静注による経験的治療を受けたが、検体を採取された 100 例で尿あるいは血液から微生物学的な感染症の所見が認められたのは 55 例のみであった。尿試験紙法陰性であったほぼ半数の患者（21 例中 10 例）では、尿培養陽性であった。この状況における診断の不確実性が、不適切な抗菌薬使用につながったことは明らかである。

監訳者コメント：
腎盂腎炎の診断における尿試験紙法の評価、および尿培養との一致・不一致は、長く検討されてきたテーマである。市中発症の腎盂腎炎で救急外来を受診した患者を対象とした本研究は、リソースと時間の限られた状況における、このテーマの診断上・検査上の課題を明瞭にしたといえる。
