Impact of moving to a new hospital build, with a high proportion of single rooms, on healthcare-associated infections and outbreaks

North Bristol NHS Trustにおいて 5 年間におけるクロストリジウム・ディフィシル（Clostridium difficile）感染症、メチシリン感受性黄色ブドウ球菌（meticillin-sensitive Staphylococcus aureus；MSSA）および大腸菌（Escherichia coli）による菌血症の発生率、ならびにノロウイルスによるアウトブレイク中の入院制限による延べ空床数減少を分析して、個室が 75％を占める新築病棟への移動により医療関連感染症の発生率が低下したかどうかを検討した。C. difficile、MSSAによる菌血症、および大腸菌による菌血症には、移動後に発生率の低下は示されなかった。ノロウイルスによる延べ空床数減少は、移動後に有意に少なかった。患者自身の保菌細菌叢に由来する場合があり空気を媒介に広範に拡散しない細菌感染症と比べて、個室を利用する可能性が高くなったことにより、空気経路を介して拡散する、感染性の高いノロウイルス感染症の伝播に対して影響を及ぼした。

監訳者コメント：
全病室数に占める個室の割合が旧病院の 10％から新病院の 50％に増加したことによる医療関連感染の発生への影響を見た論文である。菌血症の発生率に差がないものの、ノロウイルスにおいては病室の閉鎖期間が有意に短くなっているのは、個室利用による隔離が有効であったことによるが、一方で黄色ブドウ球菌や大腸菌の菌血症に差がなかったのは交差感染よりも内因性感染からの発症が原因であり、妥当な結果である。しかし、ノロとの差を空気感染経路の有無で説明している点は理解しがたいところである。医療関連感染の感染経路は接触感染経路であることから、耐性菌保菌の状況をモニターしないと、個室病室と院内伝播の変化を見ることが困難であろう。
