Randomized controlled trial of honey versus mupirocin to decolonize patients with nasal colonization of meticillin-resistant Staphylococcus aureus

背景
ムピロシンは、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の鼻腔内除菌に特に用いられるが、ムピロシン耐性の増加によりその反復使用は制限される。マヌカハニーの抗菌効果はこれまでに in vitro で確立されているが、他の蜂蜜の抗菌活性も報告されている。
目的
鼻腔内 MRSA 除菌における医療用蜂蜜の有効性を 2％ムピロシンと比較した無作為化試験から得られた経験を報告すること。
方法
MRSA の鼻腔内保菌を有する 18 歳以上の患者を募集した。参加者には、連続 5 日間の 1 日あたり 3 回の医療用蜂蜜または 2％ムピロシン投与を 1 ～ 2 コース実施した。
結果
1 ～ 2 コースの治療で除菌が示された患者の割合には、医療用蜂蜜群（42 例中 18 例、42.8％、95％信頼区間［CI］27.7 ～ 59.0）と 2％ムピロシン群（44 例中 25 例、56.8％、95％CI 41.0 ～ 71.7）で有意差がなかった。鼻腔以外の MRSA 保菌は、持続的鼻腔内保菌と有意に関連していた（オッズ比 5.186、95％CI 1.736 ～ 5.489、P ＝ 0.003）。ムピロシン耐性の新規獲得率は 9.75％であった。
結論
有意ではなかったが、医療用蜂蜜群の 42.8％という除菌率は印象的であった。本研究の結果から、抗菌薬耐性への対策となる可能性を有するこの戦略について、同様のより大規模な研究で評価すべきである。

監訳者コメント：
MRSA の鼻腔除菌に対するムピロシンと医療用蜂蜜の効果を比較した論文である。医療用蜂蜜は、クリーム製剤、ドレッシング製剤など、以前より創傷治癒などに使用されていた。ムピロシン耐性 MRSA の発生を考えると、繰り返しの投与を避けるため、医療用蜂蜜を用いるなどの可能性があると思われた。
