Assessing the burden of Clostridium difficile infections for hospitals

背景
院内感染症は、医療システムにとって重い負担をもたらす。しかし、この負担の定量化は、延長した入院期間を正確に推定する方法から、様々な医療提供者における予防法を明らかにしコストを算出することに至るまで、多くの問題を提起する。
目的
クロストリジウム・ディフィシル（Clostridium difficile）による院内感染症のコストを推定し、病院管理者に情報を提供すること。
方法
マルコフ過程に基づくマルチステート・モデリングおよびブートストラッピングを用いて、クロストリジウム・ディフィシル（C. difficile）感染症（CDI）に関連する入院の延長期間を個別に推定した。次いで、診断関連群に基づく支払システムから導出した生産性喪失の推定を含め、病院コストの指標について計算した。患者は 55 歳以上で、2013 年 1 月 1 日から 2014 年 9 月 15 日にリールの病院 2 施設に入院し、CDI のエピソードを有した者と有さなかった者であった。
結果
研究期間中の計 52 件のエピソードをスクリーニングした。CDI による入院期間延長（27 件）の推定コストは平均で約 23,909 ユーロ（SD 17,458）であった。入院期間短縮の場合（25 件）では、コストの平均は約－14,697 ユーロ（SD 16,936）であり、これが病院にとっての純節減額であった。コスト／節減の主要な誘因は、院内感染症に起因する生産性の喪失／向上であった。感度分析により、院内感染症によるコストまたは節減の額を説明する主要な因子は入院期間であった。
結論
死亡の場合における生産性の増分を、支払システムの不完全性を示す因子とする考え方について論じ、次いで時間的バイアスを補正するための統計学的方法に関連する方法論上の問題について論じた。
