水使用場所が抗菌フィルタで保護されている血液内科病棟における施設内水道配管日和見病原体の拡散経路を追跡する

Tracking the spread routes of opportunistic premise plumbing pathogens in a haematology unit with water points-of-use protected by antimicrobial filters

背景
病院内の水道設備は、施設内水道配管日和見病原体(opportunistic premise plumbing pathogens;OPPP)によって高頻度に汚染されていることから、患者の曝露を抑えるため、水使用場所に抗菌フィルタが導入されている。
目的
水使用場所 73 カ所中 52 カ所が患者へのリスクが高く、抗菌フィルタによって保護されている血液内科成人病棟において、2 次的(給水システム以外の)給水経路による OPPP 拡散を評価すること。
方法
観察的監査により、6 カ所の 2次的給水経路を同定し、表面から 315 の試料を採取して細菌の追跡調査および型別検査を実施した。マトリックス支援レーザー脱離イオン化飛行時間型質量分析法により分離株を同定し、マルチプレックス repetitive element sequence-based ポリメラーゼ連鎖反応および/またはパルスフィールド・ゲル電気泳動の制限断片長多型解析によって下位の種と比較した。
結果
抗菌フィルタの上流で採取した水で、緑濃菌(Pseudomonas aeruginosa)およびステノトロホモナス・マルトフィリア(Stenotrophomonas maltophilia)、ならびに非病原性 OPPP インジケータが検出された。緑濃菌は、検査対象の表面から検出された唯一の OPPP であった(5.1%)。緑濃菌の同一クローンが水源から水源と同室の乾燥した表面に伝播し、それぞれ別の部屋にある 2 カ所のシンク間で交差汚染が認められた。非病原性OPPPインジケータの 3 つのクローンが部屋をまたいでより広範に伝播していた。
結論
血液内科病棟の水使用場所の大半(すべてではない)で実施されているフィルタを使用した戦略は、「高リスク」の水使用場所の機能的マッピングを実施すれば、OPPP の環境への拡散制御に十分であると考えられる。フィルタが使用されていない水使用場所では、OPPP および OPPP インジケータの拡散が残存することから、フィルタを使用しない水使用については厳重なモニタリングが必要である。

監訳者コメント
本論文は水道に取り付けられる抗菌フィルタの有用性と、取り付けない場合の「水道配管日和見病原体」の拡散リスクについて報告したものである。研究対象となった病院の水道水は通常緑膿菌で汚染されていると記載があるなど、日本の病院に直接当てはめることは難しいかもしれないが、医療関連感染における水道水のリスクやその対応を考える上では参考になる論文である。
