Controlling a possible outbreak of Candida auris infection: lessons learnt from multiple interventions

背景
多剤耐性カンジダ・オーリス（Candida auris）感染は近年、5 大陸で報告されている。インドにおける侵襲性 C. auris 感染の有病率は、集中治療室で獲得されたカンジダ血症の 5.3％と推定されている。患者間、および環境から患者への C. auris の伝播は急速である。
目的
病院内での C. auris の伝播動態を理解すること、および蔓延防止のためにとりうる介入策を明らかにすること。
方法
集中治療室のサーベイランスを実施し、患者の保菌、環境汚染、および医療従事者の手指の C. auris 保菌を評価した。クロルヘキシジンによる患者の洗浄、および安定化過酸化水素消毒薬（Ecoshield）を用いた環境表面の汚染除去を含む介入を行った。さらに、院内で頻用される消毒薬の、様々な無生物表面の C. auris に対する有効性、および院内の布地上の C. auris の残存を評価した。
結果
3 例の C. auris 血流感染症が 3 か月間に見つかった。同時期に同エリアに入院していた多くの患者に C. auris の定着が認められた。サーベイランスにより、環境表面および医療従事者の手指の C. auris 汚染が認められた。クロルヘキシジンによる洗浄および消毒薬の適切な使用などの介入により、C. auris を患者および病院環境から根絶できた。
結論
当院で頻用される消毒薬、および現行の手指衛生行動は、適正な消毒薬の接触時間および手指衛生手順が遵守された場合には、C. auris に対して有効であった。乾燥状態の布地上での C. auris の残存の評価では、C. auris が最長で 7 日間残存しうることが示された。

監訳者コメント：
C. aurisは、米国でも問題となっており、米国疾病対策センター（CDC）はアラートを出している。本論文と同様に、患者間での感染伝播、環境の汚染により、医療施設内での拡散が懸念され、早期介入が必要とされている。
