Xpert Carba-R（セフィエド社）は、頻度の高い 5 つのカルバペネマーゼ遺伝子を検出および識別するための PCR アッセイである。製造者の指示に従って用いる場合、対象とする細菌分離株は血液培地またはマッコンキー寒天培地で培養しなければならい。本研究は、既知のカルバペネマーゼ遺伝子を有する様々な細菌分離株パネルに対して、同アッセイが高い性能を発揮することを確認している。本研究はまた、同アッセイが、本 PCR アッセイの製造者により推奨されていなかった 3 つの固形寒天培地、すなわち CLED（Cystine-Lactose-Electrolyte-Deficient）、ChromID CARBASMART、およびニュートリエント斜面培地を用いた場合に、高い性能を発揮することを示している。これらの培地のいずれかで直接分離株を調べることで、検査室による報告の遅れが回避可能である。

監訳者コメント：
現在、様々な菌種の様々な薬剤耐性を正確に、迅速に、そして簡便に検出する方法についての検討が行われている。セフィエド社の Xpert システム自体はすでに日本で結核菌およびリファンピシン耐性遺伝子を検出する製品が導入されている。今後も新たな検査方法が次々に製品化されることが予想されるが、各病院でそれぞれの製品を比較検討し、必要なものを選択することが重要である。
