背景
全英の自発的サーベイランスプログラムから、大腸菌(Escherichia coli)が原因の菌血症が年々増加しつつあることが示されている。2011 年に、収集されるデータの量および質を改善する目的で大腸菌菌血症の義務的サーベイランスが導入された。他の全国義務的サーベイランスとは異なり、入院との関係における発症時点に基づいた症例の分類は現在行われていない。
目的
イングランドの大腸菌菌血症症例に発症時点による分類を適用した場合を評価すること。
方法
2012 年 4 月から 2016 年 3 月に ※Public Health England に報告された大腸菌菌血症の全症例のデータを、全国義務的サーベイランスデータベースから抽出した。症例は、入院後2日目以降に血液培養陽性であった場合に病院発症型に分類した。
結果
症例の約 21%は病院発症型に分類された。しかし、病院発症型症例の割合は、連続した12 か月ごとの期間あたり 1%ずつ減少し、2012/2013 年の 23%から 2015/16 年には 20%となった(P < 0.001)。
結論
義務的サーベイランスにより報告される大腸菌菌血症症例の約 5 分の 1 は、病院発症型であると特定された。状況によって感染予防・感染制御の戦略が変わるであろうことを考えると、この情報のルーチンのフィードバックは、感染予防・感染制御の取り組みに情報を提供するうえで重要となることが示される。この症例サブセットの分類を行うことは、大腸菌菌血症の疫学をより良く理解するための重要なステップとなる。

何でも big data は集めただけではゴミ、分類すれば宝に変わる。集める前に、どういった分析を行うために情報収集するのかをよく検討した上でサーべイランスを実施すべきである。
