Systemic antimicrobial prophylaxis in burn patients: systematic review

目的
熱傷患者に対する全身抗菌薬予防投与についての研究をレビューすること。
方法
電子データベースを検索して、1966 年から 2016 年の間に、ヒトを対象に全身予防投与について抗菌薬とプラセボまたは介入なしを比較した臨床試験を特定した。
結果
19 件の試験が選択基準を満たした。熱傷後早期の予防投与を、重症でない熱傷患者（6 試験）および重症熱傷患者（7 試験）において評価した。抗菌薬予防投与には、毒素性ショック症候群または熱傷感染症の予防には有効でない（グレード 1C）が、重症熱傷患者および人工換気を要する患者では有効となり得る（グレード 2B）ことが示された。周術期の予防投与は 6 試験で評価されていた。失活組織切除時の抗菌薬予防投与は、ほとんどの熱傷患者にとって利益はない（グレード 2B）が、広範な熱傷を有する患者について推奨を行うだけのエビデンスは不十分である。抗菌薬予防投与は、選択された処置において中間層皮膚移植片の感染症予防にも効果的となり得る（グレード 2B）。
結論
現在得られるエビデンスでは、大部分の熱傷患者の管理における全身抗菌薬予防投与は支持されない。ただし、人工換気を要する重症熱傷患者、および選択された中間層皮膚移植処置において有用となる可能性がある。

監訳者コメント：
調査対象となった論文の多くは古かったり、症例数が少ないものである。2016 年 Tagamiらの論文は 412 例を検討しており参考になる（人工換気を要する重症熱傷患者の受傷後 28 日死亡率の改善が得られている）。
