The Scottish enhanced Staphylococcus aureus bacteraemia surveillance programme: the first 18 months of data in children

背景
2014 年 10 月 1 日に、スコットランドにおける黄色ブドウ球菌（Staphylococcus aureus）菌血症（SAB）の疫学についてより詳細な理解を得ることを目的として、SAB の全国強化サーベイランスが開始された。16 歳未満の小児と成人について、これまでの研究で疫学的な差が示されているため、個別に分析した。
目的
焦点を絞った改善計画の作成を可能にするため、リスク因子および最もリスクの高い患者集団を同定すること。
方法
国民保健サービス（NHS）スコットランド内の全 NHS 委員会が、義務的強化サーベイランスに参加した。データは、研修を受けたデータ収集者が、スコットランド全体で合意された定義を用いて収集した。
結果
最初の 18 か月間のデータを解析した結果、院内獲得 SAB はデバイスに関連するリスク因子を有する新生児で最も多くみられた一方、市中獲得 SAB は、リスク因子があったとしても少数で、ほとんどが骨または関節の感染症を有する、より年齢が高い小児で認められた。
結論
SAB の強化サーベイランスのデータから、小児集団におけるリスク因子の違い、および SAB の獲得経路の違いが示された。

監訳者コメント：
1 施設のデータだけでは見えないことが、エリア内の複数施設でデータを確認することで明確になることがある。本論文では、1 歳以下では院内獲得が多く、デバイスがリスク因子であったが、1 歳から 15 歳では市中獲得が多く、明確なリスク因子はほとんどなかった。
