英国における 2010/11 年から 2015/16 年の冬毎の急性胃腸炎による病床閉鎖の負荷、持続期間およびコスト

背景
急性胃腸炎による病床閉鎖により、冬がくる毎に病院はプレッシャーにさらされる。英国では、2010/11 年以来、国民保健サービス(NHS)は冬季の状況を急性期トラストすべてについてモニターしている。
目的
冬季の急性胃腸炎による病床閉鎖の負荷、持続期間、コストを推定すること。
方法
2010/11 年冬季から 2015/16年冬季について、日常的に収集した下痢および嘔吐による病床閉鎖の時系列データの後向き解析を実施した。2 つの重要なデータ上の問題は、トラストレベルで欠測値を非無作為な方法によって補完することと、6 回の冬で記録された閉鎖の日付にまで観測値にフィルターをかけることによって対処した。補完された最低値と最高値は、最良のシナリオと最悪のシナリオとみなした。病床利用に関するコストは NHS reference costs を用いて見積もり、発生しうるスタッフの欠勤コストは先行研究に基づくものであった。
結果
最良から最悪のシナリオにおいて、88,000 ~ 113,000 床(中央値)が、冬毎に急性胃腸炎により閉鎖された。これらのうち、19.6 ~ 20.4%は使用されていなかった。平均してトラストの 80%が影響を受け、冬毎の病床閉鎖は 15 ~ 21 日(中央値)であった。病床閉鎖によって病院にかかったコストは 570 万 ~ 750 万ポンドで、疾患によるスタッフ欠勤のコストを含めると 690 万 ~ 1 千万ポンドに上った。
結論
冬毎の急性胃腸炎による病床閉鎖の中央値は、英国のすべての一般および急性期病床が 0.88 ~ 1.12 日(中央値)利用できない場合と同等であった。病院のコストは高いが、冬毎の閉鎖状況に応じて変動する。

監訳者コメント:
感染性疾患において、病床閉鎖が実際にどの程度存在したかを集計するのはかなり難しい。本検討では NHS のトラストデータから推計しようと試みたが、曜日による差異や、個々の病床閉鎖期間によって決定される部分も大きかった。しかしながら最終的には、英国のすべての一般・急性期病床が約 1 日閉鎖されたのとほぼ同等となり、その負荷が非常に大きいことが判明した。医療行政、病院経営の両面で、興味深い結果であるといえる。
