米国疾病対策センターは、インフルエンザ A(H1N1-09)の検出に、最初にインフルエンザ迅速診断検査を行うことを推奨している。患者 246 例の鼻咽頭からのサンプルについて、target-enriched multiplex polymerase chain reaction(PCR)法により H1N1-09 の検査を行い、うち 163 例では合わせて迅速診断検査法による検査も行った。target-enriched multiplex PCR 法を参照基準とした時の迅速診断検査法の感度は18.7%であった。迅速診断検査法で偽陰性であった患者は、オセルタミビル投与が 111 日間、および隔離が 65 日間遅れた。target-enriched multiplex PCR 法で H1N1 陰性であった患者では、抗ウイルス療法が即座に中止され、これにより 408 日間のオセルタミビル投与、および 315 日間の不要な隔離が避けられた。これに伴って削減できた費用は 208,982 米国ドルであった。

監訳者コメント
インフルエンザの迅速診断は偽陰性が大きな問題であったが、その実態と弊害については不明であったが、本論文では PCR を基準とすると迅速診断の感度が 20%に満たないことは衝撃的なデータであり、正確な診断は無用な医療費を削減できることを示した点で、その意義は大きい。一方で自然治癒するインフルエンザに対して、高価な診断法を実施することへの抵抗もある。
