Lack of antimicrobial efficacy of mecetronium etilsulfate in propanol-based hand rubs for surgical hand disinfection

本研究は、エチル硫酸メセトロニウムが手術時手指消毒の全体的な有効性に寄与するかどうかを明らかにすることを目的に施行された。3 種類の擦式製剤（イソプロパノール 45％、n-プロパノール 30％）を盲検下で 1.5 分間使用し、EN 12791 の標準手法と比較した（クロスオーバーデザイン）。そのうち 1 つの市販擦式製剤は 0.2％エチル硫酸メセトロニウムを含有し、残り 2 つの擦式製剤は 0.2％エチル硫酸メセトロニウムを含有する以外は同一成分の製剤であった。いずれの擦式製剤も、標準手法と比較して 3 時間後の菌数（log₁₀）は有意に減少しなかった（平均 1.72［標準偏差 1.15］）。擦式製剤に含有されるエチル硫酸メセトロニウムが抗菌効果に寄与しているかどうかは疑問である。

監訳者コメント：
エチル硫酸メセトロニウム（MES）は界面活性剤に分類され、非特異的な抗菌効果を期待し、欧州・中東・インドで擦式手指消毒薬に含有されている。非揮発性である点は、皮膚細菌叢への微生物の再付着を遅らせる意味から手術時の手指衛生に有利とされてきたが、今回の検討では 3 時間観察しても MES による菌数の付加的減少効果は認められなかった。MES が多量に用いられている国もあり、環境への影響を減らし、抗微生物効果を保つ意味でも、使用制限に踏み切った方がよいと筆者は考察している。
