Investigation of outbreaks of Pneumocystis jirovecii pneumonia in two Scottish renal units

ニューモシスチス・イロベチイ（Pneumocystis jirovecii）は日和見病原体として認識されている。近年、P. jirovecii のヒトからヒトへの伝播が実証されている。しかし、P. jirovecii 伝播を促進する疫学的背景が十分に理解されていないため、P. jirovecii 感染症のアウトブレイクは十分に明らかにされていない。本稿では、西スコットランドの腎移植患者における P. jirovecii 肺炎の 2 つのアウトブレイクを報告する。地理的に隣接した 2 つの場所で、患者計 25 例が感染した。主要タイプとしてアレル B、ならびにアレル A3 が同定された。症例のクラスタリングの正確な理由を明らかにすることはできなかったが、1 つのアウトブレイクでは外来クリニック環境が注目された。スルファメトキサゾール・トリメトプリムの予防投与により両アウトブレイクは終息した。予防投与を受けた標的集団は、2 つのアウトブレイクで異なっていた。感染制御チームは、P. jirovecii 肺炎アウトブレイクの可能性について警戒すべきである。

近年ニューモシスチス・イロベチイのアウトブレイクが様々な環境で報告されており、システマティックレビューなども行われるようになっている。患者を（陰圧）個室に入室させたり、少なくとも免疫抑制患者との同室は避けるよう指導している専門家もいる。リスクのある患者には ST 合剤の予防投与を行うことも重要である。
