Universal screening and decolonization for control of MRSA in nursing homes: follow-up of a cluster randomized controlled trial

2010 年から 2011 年にナーシングホームで行った試験では、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の保菌率の低減において、MRSA の普遍的スクリーニングと除菌に、標準的予防策を上回る利益は認められなかった。このため、2012 年以降ルーチンのスクリーニングは実施されなかった。5 年間のフォローアップでも、介入を支持する新たな根拠は示されなかった。試験後に発表した推奨（入所者に対する MRSA のスクリーニングと除菌は行わない）を維持する。

監訳者コメント：
ナーシングホームなどの介護施設や長期療養型病床における MRSA 感染対策をどこまでするのかは、先進国における大きな問題である。著者は以前にナーシングホームの入所者の MRSA スクリーニング（鼻腔、咽頭、陰部）と除菌の無作為化比較試験を実施し、介入群（51 施設）と対照群（53 施設）とに MRSA 保菌率に差が認められなかったことを報告（Infect Control Hosp Epidemiol. 2015 Apr;36(4):401-8.）し、2012 年以降これらの介入を中止した。今回はその後の長期経過観察後の結果でも介入による新たな知見がないことを確認した。すなわちナーシングホームでのスクリーニングと除菌は MRSA 保菌率に影響しない。
