Identification badge lanyards as infection control risk: a cross-sectional observation study with epidemiological analysis

名札のストラップの培養で得られた黄色ブドウ球菌（Staphylococcus aureus）株は、着用者の鼻腔内保菌株と比較すると、パルスフィールド・ゲル電気泳動による遺伝子型およびアンチバイオグラムによる表現型は区別不能であり、同一クローンの可能性が高い。ストラップの使用期間は平均 22 か月であり、衛生状態は不良で、洗浄されていたのはわずか 9％であった。分子疫学的解析から、黄色ブドウ球菌鼻腔内保菌者の 26％には共通して、ストラップから同一パターンの分離株が認められた。ストラップは臨床医療現場のスタッフには推奨すべきではない。

監訳者コメント：
英国保健省の指針には、誰が自分をケアしてくれているかを患者がわかるよう医療従事者全員がユニフォーム着用および／または認識票を身に付けていることとされている。ネックストラップは、洗濯することがないまま継続使用されているが、2 か月に １ 回は洗濯すべきとしている論文もある。本論文ではストラップから分離された黄色ブドウ球菌の遺伝子型別を実施し、4 分の １ の例で鼻腔と同一菌株であることが判明し、鼻腔保菌が供給源となっている可能性が示唆される。感染予防の目的から英国 NHS はネクタイ着用を推奨していない。しかしながら、これらの事実は、必ずしも感染と直接関連しているかは不明であり、菌検出即使用禁止とはいえない。これまで、ネクタイや聴診器などで同様の研究報告があるが、アウトブレイクとの直接証明できたものはない。ストラップは定期的に洗濯するかまたは、名札をクリップで留めることが良さそうだ。
