Attributable mortality of hospital-acquired bloodstream infections in Ireland

目的
アイルランドにおける病院感染による血流感染症の寄与死亡率を推定すること。
方法
2007 年 1 月から 2013 年 12 月までの間、アイルランドの 6 つの病院の微生物検査室からの届け出と患者管理記録をもとに、後向き症例コホート研究を実施した。確率的リンケージにより、入院患者 343,189 例のコホートから、血流感染症の症例 1,252 例のデータを連結した。多変量ロジスティック回帰モデルを用いて、独立した死亡の予測因子を同定した。予測因子は、患者の年齢、緊急入院か再入院か、集中治療室への入室日数、手術回数、診断名数、主要診断群、および病院感染による血流感染症の有無などであった。
結果
症例被験者の粗死亡率から、他の死亡予測因子で調整して算出した寄与死亡率は 15.3%(95%信頼区間 14.8 ~ 15.8)であった。本研究ではさらに、原因微生物による層別化も行った。原因微生物には、大腸菌(Escherichia coli)、エンテロコッカス・フェシウム(Enterococcus faecium)、エンテロコッカス・フェカーリス(Enterococcus faecalis)、緑膿菌(Pseudomonas aeruginosa)、肺炎桿菌(Klebsiella pneumoniae)、黄色ブドウ球菌(Staphylococcus aureus)、および肺炎球菌(Streptococcus pneumoniae)を含め、データがあればそれらの抗菌薬耐性パターンを含めた。これらの微生物のうち寄与死亡率が最も高かったのは E. faecium の 18.1%であり、最も低かったのは大腸菌の 13.6%であった。一部の微生物の抗菌薬耐性パターンで有意に高い寄与死亡率が認められ、中でもメチシリン耐性黄色ブドウ球菌は 19.5%(メチシリン感受性黄色ブドウ球菌は 13.3%)であった。
結論
病院感染による血流感染症は死亡の重要な原因であり、寄与死亡率は、原因微生物および抗菌薬耐性パターンにより有意に異なる。
