Resource impact of managing suspected Middle East respiratory syndrome patients in a UK teaching hospital

メッカ巡礼からの帰国者のケースシリーズ研究および費用分析レビューでも示されたように、中東呼吸器症候群コロナウイルス（MERS-CoV）感染の可能性を有する英国への帰国者の入院患者の管理には、特に臨床像がインフルエンザと重複するという点で、臨床的な課題が存在する。これらの患者は、急性熱性呼吸器疾患により入院し、当初は MERS-CoV 感染の疑いとして管理されたが、最終的にインフルエンザと診断された。追加的な費用は少額であったが、感染予防策の強化により、隔離室およびスタッフの時間に重大な負担が生じた。メッカ巡礼のような予測可能なイベントを考慮することは、リソース管理のために重要である。本研究では、院内 MERS-CoV 診断検査が可能であれば、より早期の診断および退院が容易になったであろう。

監訳者コメント：
MERS-CoV 感染やエボラウイルス感染が疑われる場合に、例えばインフルエンザやマラリアなどが診断された場合でも、「では MERS-CoV やエボラウイルス感染が重複感染していないと言えるのか」という問題が残る。確かに院内で MERS-CoV やエボラウイルス感染に関する検査ができれば、より早期に不要な感染対策は中止できるとも考えられるが、ではどのタイミングで、何回陰性なら本当に感染対策を中止できるのか、どのくらい症例数があれば費用対効果が見合うのか、どのような病原体の検査ができればいいのか、など実際に運用するには色々と検討が必要ではないだろうか。
