Correlation of nosocomial bloodstream infection incidences: an ecological study

この研究は、感染制御活動による環境面からの影響と院内血流感染症発生率との経時的な相関を見いだすことを目的とした。ある大学病院の血液内科病棟と集中治療病棟に48時間以上にわたって入院して、2004年1月1日から2006年6月30日に退院した患者全例を対象としてプロスペクティブに調査した。院内血流感染症の症例定義は、（1）通常の病原体の場合は血液培養が1回以上陽性かつ臨床的徴候があること、（2）コアグラーゼ陰性ブドウ球菌、バチルス（Bacillus）属（ただし、炭疽菌［Bacillus anthracis］を除く）、コリネバクテリウム（Corynebacterium）属、プロピオニバクテリウム（Propionibacterium）属、ミクロコッカス（Micrococcus）属、あるいは同等の病原性を有するその他の常在菌の場合は、48時間以内に同一の病原体が血液培養で2回以上陽性であることとした。3か月ごとの院内血流感染症発生率の相関をSpearman検定および直線回帰により評価した。合計で、対象患者数は3,829例、リスクのある延患者数は46,474患者・日であった。院内血流感染症と判定された患者数は集中治療病棟101例、血液内科病棟286例であった。院内血流感染症発生率は集中治療病棟と血液内科病棟の間に有意な相関がみられた（r ＝ 0.68、P ＝ 0.042）。病棟間の院内血流感染症発生率の線形モデルではR²＝ 0.52であり、正のトレンドが認められた（P ＝ 0.029）。衛生対策の改善などの両病棟に共通する決定因子がこの関連に影響を及ぼした可能性が最も高い。

監訳者コメント：
患者背景が異なり、物理的に離れた部署にあり、医療行為の内容や環境が異なる2つの病棟において、医療関連血流感染症の発生率がパラレルに推移することが報告された。フランスの大学病院からの報告であるが、感染制御チーム（ICT）による全病院的な手指衛生に関する教育プログラムが有効であったのではないか、と議論されている。
