Management and cost of surgical site infection in patients undergoing surgery for spinal metastasis

背景
手術部位感染（SSI）は、重篤となる可能性のある脊椎手術合併症である。SSI は入院患者の入院期間および追加の治療に伴う費用に大きな影響を及ぼす可能性がある。
目的
脊椎転移性腫瘍に対する手術後に SSI がみられた患者の管理方法を検討し、これに関連した SSI に伴う費用を推定すること。
方法
2009 年 1 月から 2012 年 12 月に大規模な脊椎手術施設で脊椎腫瘍手術後に SSI が認められた患者を同定した。既存の症例記録を再検討し、患者および手続き上のデータ、感染の詳細、治療介入を記録した。SSI が認められた患者に対し、感染に伴う費用を算出するためのボトムアップ・アプローチを用い、SSI が認められない同様の患者の擬似ランダムサンプルと比較した。
結果
SSI 患者の治療費用の平均は、非 SSI 患者より有意に高かった（P ＝ 0.019）。入院患者の入院費用の平均は、SSI 患者のほうが非 SSI 患者と比較して 60％高かった（P ＝ 0.004）。入院患者の入院単独で全費用の 59％を占めた。再手術は、全体で 2 番目に費用の高い介入で、全費用の 38％を占め、SSI治療に関連したもっとも費用のかかる単独の介入であった。
結論
SSI によって、脊椎転移に対する手術を受ける患者の医療費は、入院患者の入院期間の延長と、創傷管理のための再手術を主な要因として、大幅に増加する。この患者群において、SSI に由来する、社会への実際の総費用は医療提供者への単なる直接経費をはるかに超える可能性が高い。

ボトムアップアプローチはデータを一つずつ分析しながら価値判断を積み上げていく経済分析の手法である。本論文では、集まったデータをそれぞれに分析し、SSI 患者と非 SSI 患者で比較を行った。入院費や再手術などがどれぐらいの負担の増大になるのかが明確になっており、説得力がある。
