ICU ward design and nosocomial infection rates: a cross-sectional study in Germany

背景
病院および病棟の設計が多面的な感染制御に及ぼす影響について関心が高まっている。確定的なエビデンスはまれであり、現在の病棟設計についての知識は不足している。
目的
集中治療室（ICU）の病棟設計の現状に関するデータを収集し、特定の設計要因と院内感染率の関連を分析すること。
方法
2015 年、ドイツの院内感染サーベイランスシステム（KISS）に自発的に参加した ICU からオンラインの質問票により運営設備に関するデータを収集した。その後、2014 年から 2015 年の KISS のデータベースから得た院内感染率を用いて、多変量解析を実施した。
結果
全体で、534 の ICU から運営設備のデータが得られた。このうち、27.1％のベッドが中程度の大きさ 18 m²（四分位範囲 15 ～ 21 m²）の 1 人部屋で使用され、すべての ICU のベッドの 73.5％は、近くに擦式手指ディスペンサーを備えていた。著者らは、多変量解析において 266 の ICU をマッチさせることができた。病室に開閉可能な窓のある ICU は、機器に関連した下気道感染が少ないことと関連していた（オッズ比［OR］0.73、95％信頼区間［CI］0.58 ～ 0.90）。2 つのベッドを備えた部屋が40％を超えるICUは、主要な血流感染率が低いことと関連していた（OR 0.66、95％CI 0.51 ～ 0.86）。
結論
設計要因と ICU 感染率にはわずかな関連しか認められなかった。大半は他のリスク因子の代用であった。

監訳者コメント：
環境が施設内感染に及ぼす影響について、近年関心が高まっている。本論文では明らかな結果はでなかったものの、このような研究が今後も増えていくものと思われる。
