Manual resuscitators in successive use in the same patient: reservoir of multi- and extensively resistant bacteria

手動蘇生器上の細菌量を明らかにするため、ブラジルの集中治療室 1 施設において手動蘇生器のコネクタのスワブ試料を、0 時間目ならびに 同一患者での 24 および 48 時間の使用後に採取した。20 個の手動蘇生器から、計 54 の細菌が分離され、t ＝ 0 で 6、t ＝ 24 で 17、t ＝ 48 で 31 であった。黄色ブドウ球菌（Staphylococcus aureus）、アシネトバクター・バウマニー（Acinetobacter baumannii）、セラチア・マルセセンス（Serratia marcescens）および緑膿菌（Pseudomonas aeruginosa）が主な分離株で、すべての細菌属において多剤耐性が広範に認められた。患者間での手動蘇生器の効果的なコンタミネーション除去を確実に行うこと、ならびに個々の患者においてより頻繁に手動蘇生器を交換することへの認識を高めることが、気道への（再）保菌／感染のリスクを抑えるために推奨される。

アンビューバッグの汚染リスクは以前から指摘されているが、同一患者に繰り返し使用するうちに、その細菌汚染も経時的に高度になることが示された。このことは例え同一患者に使うのであっても、アンビューバッグは適宜交換しなければならないことを示唆している。各施設におけるアンビューバッグの取り扱いを今一度見直すと良いだろう。
