Clinical value of stool culture in paediatric oncology patients: hospital evaluation and UK survey of practice

下痢は小児腫瘍患児に頻繁に生じる症状である。下痢を来した入院患者にルーチンに腸内細菌検査を行うことは、一般的にはあまり有用ではない。しかし小児腫瘍患児においてはその有用性に関する検討は報告されていない。そこで我々は、当 3 次小児腫瘍科において 5 年間の後向きの解析を行うと共に、21 施設からなる全国調査を実施し、下痢を呈する腫瘍患者の便培養の有用性と、英国内の取り組みを検討した。当施設の調査ではルーチンの便培養によって診断に至った割合は極めて低く、検査陽性は 842 件中わずか 1 検体（0.1％）であった^※。全国調査では実施にかなりのばらつきが認められた。当施設では小児腫瘍患児を対象とした従来法による腸内細菌の便培養には、エビデンスはほとんどないといえる。以上の結果は、国内の検査指針の策定に際し情報として提供されるべきである。

本検討は小児腫瘍科の受診患児で、下痢を来していた例を対象としており、入院例・日帰り入院例を含む。便培養の対象菌についての詳細な情報はないが、クロストリジウム・ディフィシル（Clostridium difficile）検査は 2 歳を超え、下痢が続き、便に血液ないし粘液を伴い、腸管炎症の強い例のみについて行っている。ルーチンではなく、患者背景と目的を考慮した培養検査の適切さ・重要性を確認した結果といえる。
