Staphylococcus aureus and surgical site infections: benefits of screening and decolonization before surgery

手術部位感染（SSI）は、もっとも頻度が高い医療関連感染のひとつであり、患者の罹患率と医療費に大きな影響を及ぼす。黄色ブドウ球菌（Staphylococcus aureus）はもっとも一般的な原因菌である。黄色ブドウ球菌の疫学は、新しいクローンの拡散やムピロシン耐性の出現により変化している。SSI の予防と管理は多面的であり、本稿では、術前の黄色ブドウ球菌鼻腔内保菌者のスクリーニングと続いて行う陽性患者の除菌の、有用性に関するエビデンスをレビューした。培養または遺伝子検出を用いて術前スクリーニングを実施し、続いてメチシリン感受性黄色ブドウ球菌およびメチシリン耐性黄色ブドウ球菌（MRSA）陽性患者の除菌を行うことにより、SSI および在院日数は減少する。これは、心胸郭手術や整形外科手術のような、植込み用のデバイスを挿入する清潔手術（大手術）にとくに当てはまる。しかし、これらの施行には、患者の教育に加えて集学的アプローチが必要である。黄色ブドウ球菌のスクリーニングを実施せずに全患者の除菌を術前に行うのは、黄色ブドウ球菌の新しいクローンの出現を監視できなくなったり、ムピロシン耐性の一因となったり、MRSA を考慮した術前の予防的抗菌薬（すなわちβ-ラクタム系薬をグリコペプチド系薬などに置き換えること）を適切に選択できなくなる恐れがある。

術前の黄色ブドウ球菌のスクリーニングとその応用・有用性・限界については、これまで多くの報告と議論がなされてきた。加えて遺伝子検出法が進歩し、汎用性・迅速性・実用性の点で向上したことも、スクリーニングの評価を変えつつある。本稿はその最新情報をレビューするものであり、一読の価値がある。
