Optimizing antibiotic usage in hospitals: a qualitative study of the perspectives of hospital managers

背景
病院における抗菌薬の最適化は、急増する耐性に関連して、非常に重要であり、ますます優先度が高まっている。病院内での抗菌薬使用を最適化するためには、医師重視にもかかわらず、非処方者を含むさまざまな関係者がどのように診療や診療の変更に影響を及ぼすかを理解する必要がある。
目的
今回の研究は、オーストラリアの病院管理者の、抗菌薬耐性、抗菌薬ガバナンスの管理、経営上の優先順位に対する臨床上の優先順位の折り合いに関する視点を理解するために計画した。
方法
2014 年および 2015 年、オーストラリアの 3 病院の管理者 23 名が定性的半構造的面接に参加した。データは系統的に符号化され、テーマに関して分析された。
結果
経営上の視点からの結果は以下のものである。（1）相反する要求が抗菌薬ガバナンスの優先順位付けの支障となる、（2）監査およびモニタリング方法に効果が不十分なため、変更に対する合理的説明が制限される、（3）医師に対する臨床的教育およびフィードバックが少ない、（4）経営側に向けられた変更プロセスは、医師の間に抗菌薬使用に関して認識された「説明責任を明確にする文化」がないことに制約されている。
結論
病院管理者は、抗菌薬の最適化およびガバナンスの実現に向けて考慮すべき構造的問題や専門職間の問題を報告している。これらの問題は、病院において管理者が直面する他の問題と比較して最適化の優先順位を低くしており、管理者に変化の理解と、変化への障害への取り組みを促す抗菌薬適正使用支援プログラムの重要性を示している。
