Healthcare worker and family caregiver hand hygiene in Bangladeshi healthcare facilities: results from the Bangladesh National Hygiene Baseline Survey

背景
医療施設における手指衛生は、患者ケア、医療従事者の安全、および感染制御に影響を及ぼすが、低所得国には介入の指針となるデータがほとんどない。
目的
バングラデシュの医療施設において、手指衛生のインフラおよび手指衛生行動に関する全国的調査を実施し、政策の助けとなるベースラインのデータを確立すること。
方法
2013 年の Bangladeshi National Hygiene Baseline Survey において、家庭、学校、飲食店および食品屋台（food vendor）、伝統的助産師ならびに医療施設における、水、衛生設備、および手指衛生を調査した。我々は確率比例抽出法に基づき、農村部から 50、都市部から 50、合計 100 の人口集団を抽出した上で、875 の入院施設で手指衛生インフラを調査し、100 施設で行動を観察した。
結果
96％を超える施設が「改善された」水源を持っていたが、多くの場合、水源の周囲環境で汚染が発生していた。石けんは、医師および看護師の手洗い場の78 ～ 92％に設置されていたが、患者および家族が使用する手洗い場で設置されていたのは4 ～ 30％にすぎなかった。アルコール製消毒薬の使用または石けんによる手洗いと、空気乾燥または清潔な布による乾燥という、推奨されている行動が取られていたのは、4,676 回の手指衛生機会のうちわずか 2％であった。医療従事者は、919 回の機会のうち 9％で推奨されている手指衛生を実践しており、この割合は、患者との接触の前（11％）よりも後（26％）のほうが高かった。家族介護者は水のみによる手洗いが多く（2,751回の機会のうち 48％）、石けんの使用は少なかった（3％）。
結論
医療従事者は家族よりも、手指衛生器材が利用しやすく、手指衛生の実践も良好であったが、それでもなお遵守率は低い。手指衛生器材および手指衛生行動を増やすことで、バングラデシュの医療施設における感染制御が改善されるかもしれない。

本研究はバングラデシュにおける手指衛生実践の状況を、利用環境と利用状況の双方で調査したものである。単なる手指衛生の現状調査にとどまらず、途上国の衛生の向上および増進を図るうえでの政策立案に活かす点が興味深い。今後、本研究を基に立案・実行された政策が実際におさめた効果について、報告が待たれるところである。
