背景
インフルエンザウイルス感染患者の迅速な同定、正確な症例定義、および厳格な衛生対策は、院内伝播の予防に重要である。
目的
インフルエンザ患者を迅速に同定するための症例定義の有用性を示すこと、ならびに病院スタッフによるサージカルマスクの常時着用を含む 2 段階の衛生管理が院内感染率に及ぼす影響を検討すること。
方法
2015 年 1 月から 3 月までの間にインフルエンザの疑いで入院した全患者を登録した。リアルタイム PCR 法によるインフルエンザ検査を実施した。病院スタッフが感染患者と密接な接触を行う間にサージカルマスクを着用することを含む、国が定める衛生ガイドラインに準じた感染患者の衛生管理を行った。インフルエンザの活動性が高まった時には追加対策として、病院スタッフによるサージカルマスクの常時着用を実践した。
結果
本研究に登録した患者の多くは高齢者であった(212 例、平均年齢 75 歳)。呼吸器感染の臨床的指標でインフルエンザ陰性患者と陽性患者で異なったのは、咳の頻度のみであった。感染患者との密接な接触時に対象を絞ったサージカルマスクの着用と比較すると、勤務時間を通したサージカルマスクの常時着用により、院内感染は減少した(発症率はそれぞれ 31%、16%、P < 0.01)。
結論
高齢の入院患者において、インフルエンザ A 型と他の呼吸器感染の臨床的特徴に基づく鑑別は不可能であった。衛生管理に関しては、病院スタッフによるサージカルマスクの常時着用は院内感染の予防に有効であるようである。

シーズン初期はインフルエンザ発症者に対応するときの飛沫予防策、流行時期になったら勤務時間中継続したサージカルマスクの着用という 2 段階法は、リーズナブルである。ただし、職員が「きちんとマスクを付けることができる」(鼻マスク、あごマスクをしない)が大前提であり、ここが難しいところでもある。
