緑膿菌(Pseudomonas aeruginosa)は重要な院内感染病原体であり、蛇口や流し台を含めた院内給水システムに定着する。本稿では、5 か月の期間中に救命救急病棟の同一の個室型熱傷病室に入院していた患者における水道水からの緑膿菌獲得のクラスター発生について報告する。患者 4 例からの培養で得られた臨床分離株の緑膿菌は、パルスフィールド・ゲル電気泳動により、水道水由来の分離株と識別できなかった。国内ガイダンスに従っていたにもかかわらず、救命救急病棟の蛇口が緑膿菌の伝播源であった可能性があり、患者への伝播リスクを低減するためにはガイダンスの更新と感染制御策の改善が必要である。

監訳者コメント:
水道の蛇口は無菌的ではないので、手洗いの際は使用する日の最初は暫く放水するのが望ましい。バイオフィルム産生菌がこうした水周りに定着すると除染が困難な場合もしばしばあり、蛇口の交換を余儀なくされる場合もある。
