New genotyping method discovers sustained nosocomial Pseudomonas aeruginosa outbreak in an intensive care burn unit

背景
緑膿菌(Pseudomonas aeruginosa)は集中治療室(ICU)における医療関連感染症の主要な原因である。
目的
2 年間にわたり ICU で臨床検体から回収された緑膿菌の原因不明の検出率上昇を検討すること。
方法
従来のツールを用いた疫学的調査が失敗した後、2010 年 1 月から2012 年 7 月に入院した全患者の緑膿菌臨床分離株について、新規の double-locus sequence typing(DLST 法)によりタイピングを行い、研究期間中に回収された環境分離株と比較した。
結果
合計で、患者 218 例の臨床分離株 509 株および環境分離株 91 株のタイピングを行った。患者 218 例中 154 例(71%)で 35 の遺伝子型クラスターが見いだされた。最大のクラスターである DLST 1-18 には、ほとんどが熱傷病棟での入院期間が重なる患者 23 例が含まれた。遺伝子型 DLST 1-18 の分離株は、水治療室のフロアトラップ、シャワートロリー、およびシャワーマットレスからも回収され、熱傷病棟の環境汚染がアウトブレイクの原因であることが示唆された。適切な感染制御策の実施後、この遺伝子型の分離株は熱傷患者 1 例の臨床検体で 1 回、環境検体で 2 回だけ回収されたが、その後 12 か月の追跡調査期間には回収されなかった。
結論
新規の DLST 法の使用により、大量の臨床および環境分離株の遺伝子タイピングが可能になり、熱傷病棟における原因不明の大規模アウトブレイクの環境発生源の特定に至った。ICU における緑膿菌クローンの継続的な疫学的サーベイランスの実施後に、アウトブレイクの根絶が確認された。

監訳者コメント
遺伝子多型性を検出する技術の標準は PFGE 法であるが、時間と手間がかかるので代替法として POT 法や MLST 法や本法が開発されている。しかしシークエンスはどの医療機関でもできるわけではないので、対応性としてみると病院実務としては劣る点がある。
