Whole-genome sequencing as standard practice for the analysis of clonality in outbreaks of meticillin-resistant Staphylococcus aureus in a paediatric setting

メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）は、病院関連感染症の主要原因の1つである。本研究では、小児科入院患児で発生した過去のアウトブレイクに関連した MRSA 株を再検討することにより、サーベイランス目的での全ゲノムシークエンシングの使用の可能性を検討した。全ゲノムシークエンシングデータにより、以前に Sanger シークエンシングおよびパルスフィールド・ゲル電気泳動で得られた遺伝子配列プロファイルが改善され、アウトブレイクの関連株と非関連株が識別された。これにより、株のクローン性を、以前に達成されたよりも高いレベルの解像度で確定できた。本研究により、全ゲノムシークエンシングを用いて病院アウトブレイクを追跡できる可能性が示され、これにより全ゲノムシークエンシングがアウトブレイク調査における標準法となる可能性がある。

監訳者コメント：
全ゲノムシークエンシングにより、より細かな遺伝子変異も検出することができるようになる。しかし細菌は一定の確率で突然変異を繰り返すため、特にアウトブレイク調査の時に「同一の株かどうか」を判定する基準はまだ確立していない。全ゲノムシークエンシングにより、菌の様々な毒性因子なども同時に分かるなどの利点は多いが、その結果を感染対策にどのように活用するかについては、より慎重な議論が必要だろう。
