脳脊髄液シャント感染は、脳脊髄液シャント留置の合併症であり、重篤で壊滅的となりうる。シャント感染の重要なリスク因子は、若年の脳脊髄液シャント感染歴または再留置歴、シャントの種類である。半数を超える症例で、黄色ブドウ球菌（Staphylococcus aureus）およびコアグラーゼ陰性ブドウ球菌が原因となる。その病因において、バイオフィルムが中心的な役割を果たしている。脳脊髄液培養は依然として脳脊髄液シャント感染診断のゴールドスタンダードである。脳脊髄液シャント感染予防の最も有効な方法は、滅菌方法の最適化と、適切かつ適時の抗菌薬予防投与である。脳脊髄液シャント感染の管理では、シャントのすべての部品の除去、外部装置の一時的留置、静注抗菌薬の投与と、その後のシャント再留置が必要になることが多い。本総説では、脳脊髄液シャント感染のこれまでの報告結果をまとめ、分析するとともに、この重要な事象の予防および管理について検討する。

脳脊髄液シャントも体内挿入異物でありプラスチック製のチューブは病原菌の産生するバイオフィルムの温床となるため、シャント感染時にはチューブの抜去が治療の上での鍵を握る。バイオフィルム産生菌によるシャント感染の場合、治癒せしめるためにはチューブの抜去と抗菌化学療法の組み合わせが重要である。
他の体内挿入デバイス関連感染と同様人工の異物である脳脊髄液シャントのチューブはバイオフィルム感染の温床となる。従って長期留置目的の脳脊髄液シャントチューブの留置を行う際には衛生的な挿入手技の遵守が重要である。
