マトリックス支援レーザー脱離イオン化飛行時間型質量分析法を用いて、血液培養ボトルから分離された腸内細菌科細菌について、第 3 世代セファロスポリンに対する耐性を容易に検出できるプロトコールを開発し、その評価を行った。
作業時間を最小限とし、ルーチンのワークフローに組み組むため、抗菌薬ディスクを置いたチョコレート寒天培地に発育したコロニーを用いた(菌数の標準化は行わない)セフォタキシム加水分解アッセイを開発した。このアッセイは、セフォタキシム感受性株とセフォタキシム耐性株の識別に良好な結果を示し、大腸菌(Escherichia coli)で優れた結果が得られた(感度 94.7%、特異度 100%)。しかし、AmpC 遺伝子を発現する腸内細菌科細菌や、カルバペネマーゼ産生肺炎桿菌(Klebsiella pneumoniae)におけるセフォタキシム耐性については信頼性をもって検出されなかった。

マトリックス支援レーザー脱離イオン化飛行時間型質量分析法(MALDI-TOF)は迅速性・簡便性に優れ、菌種の同定に多く用いられ、さらに薬剤耐性の検出にも応用されつつある。腸内細菌科において第 3 世代セファロスポリン耐性が迅速に判明すれば、治療・感染対策の両面において利用価値が高い。今回の検討では限界を示す結果であったが、菌種・耐性機構によってどの程度差があるのか、今後の知見が待たれる。
