幹細胞移植後のパラインフルエンザ3型の感染:高罹患率だが死亡率は低い

パラインフルエンザウイルス3型(PIV 3)は幹細胞移植後の気管支疾患の原因としてよく知られており、推定発生率は2~7%で、下気道感染による死亡率が高い。幹細胞移植レシピエントに認められたPIV 3陽性23例の12カ月の後向き研究を実施した。感染頻度は、適合非血縁ドナーの幹細胞移植レシピエント36.1%、同胞同種幹細胞移植レシピエント23.8%、自家移植レシピエント2.3%であった。標準的な感染制御対策にもかかわらず、17例は外来患者または市中感染例であった。患者11例は、上気道感染の症状のみであった。下気道感染の症状が12例の患者で発現し、そのうち8例に胸部X線検査で新たな浸潤影が認められた。PIV 3診断後の30日総死亡率は4%(患者1例)であった。PIV 3診断後100日以内に4例の患者が死亡したが、PIV 3はすべての死亡患者で死亡の主要原因ではないと考えられた。患者8例には早期にリバビリンが使用されたが、リバビリンを投与された患者で死亡したのは1例だけであった。これらの結果は、特に同種幹細胞移植レシピエントにおいて、PIV 3の罹患率は以前の報告より高いが、死亡率は低いことを示唆している。高い罹患率は、移植部門の呼吸器系ウイルスに対する積極的監視培養の方針、および特に集団発生期間の外来部門におけるPIV 3伝播抑制の困難さを反映していると著者らは考える。リバビリンによる治療は下気道感染患者の転帰を改善するかもしれないが、すべてのPIV 3感染患者に必要ではない。
