救急部における末梢静脈カテーテルの挿入に関連した静脈炎の予測因子としての看護ケア:前向き研究の結果

背景
現時点で、救急部における末梢静脈カテーテル(PVC)の挿入に関連した静脈炎の発生率を調査した研究はほとんどない。
目的
救急部で留置された PVC の挿入部のその後の経過や、PVC 関連静脈炎の発生率・重症度を調査し、患者、PVC、看護ケア因子の関連を明らかにすること。
方法
救急部で緊急性の高い症例として治療を受けた後、病棟に 24 時間以上滞在した患者 1,262 例を対象に、前向き研究を実施した。最初に挿入された PVC を抜去まで毎日観察し、Visual Infusion Phlebitis Scale を用いて静脈炎を評価した。患者、PVC、看護ケア、病院組織に関して変数を設けてデータを収集し、イベント発生までの時間事象分析を実施した。
結果
PVC 関連静脈炎の有病率(ある一点での静脈炎の罹患率)は 31%であった。累積発生率は、挿入後 3 日の時点でほぼ 20%(391 例中 78 例)であり、挿入後 5 日の時点で 50%超(391 例中 231 例)に達した。専門的な病院での滞在(ハザード比[HR]0.583、95%信頼区間[CI]0.366 ~ 0.928)、およびより多くの看護ケアを受けること(HR 0.988、95%CI 0.983 ~ 0.993)は、すべての時点で、PVC関連静脈炎に対して予防的にはたらいていた。一方で看護ケアを実施しないと、PVC関連静脈炎の新規発生率は約 4%(HR 1.038、 95%CI 1.001 ~ 1.077)上昇した。
結論
PVC 挿入後の患者に対する看護ケアの実施の有無、および看護ケアに関する専門知識の有無は、静脈炎発生率に影響を与えた。また、より多くの看護ケアを受けること、および専門的な病院での滞在は、PVC 関連静脈炎のリスク低下と関連した。これらは静脈炎の発生に関して、修正可能なリスクファクターであり、病院と病棟の双方のレベルで介入可能な領域であることを示唆している。

監訳者コメント
本研究では、PVC 挿入後の患者へのケアによって、静脈炎の発生リスクを低下させる程度や、介入できる因子について明らかにすることができた。今後の詳細な検討での成績や、具体的な事例での介入内容と減少率について報告が待たれるところである。
