Therapeutic drug monitoring of cerebrospinal fluid vancomycin concentration during intraventricular administration

髄膜炎に対するバンコマイシンの脳室内投与については、入手されるデータに限りがある。本研究では、脳脊髄液濃度と相関する臨床的特性を探索した。9 年間にわたり、患者 13 例の脳脊髄液のバンコマイシン濃度を 34 回評価した。脳脊髄液の排出量および投与からの時間と脳脊髄液のバンコマイシン濃度との間に相関が認められた。脳脊髄液中の蛋白質、白血球数またはグルコースについては関連は認められなかった。

監訳者コメント：
メチシリン耐性黄色ブドウ球菌などによる髄膜炎に対してバンコマイシンが使用されるが、バンコマイシンは髄液移行性が不良であり、しばしば全身投与に加えて髄腔内投与が行われる。しかしバンコマイシンの髄腔内投与における薬物体内動態は明らかになっていない。本研究では脳脊髄液の産生量や、バンコマイシン投与からの経過時間などが脳脊髄液中のバンコマイシン濃度と相関することが明らかになった。実際にバンコマイシンの髄腔内投与を考慮しなければならない状況は少ないと思われるが、そういった際には薬剤師の協力を得て本研究結果を参考にできるかもしれない。
