Historical outbreak of Salmonella hadar

背景
本稿では、産科におけるサルモネラ・ハダー（Salmonella hadar）の歴史的アウトブレイクについて報告する。感染初発患者の入院後にアウトブレイクが発生し、3 か月間にわたって周産期母子部門の他の 11 例への伝播が生じた。
結果
診療法の厳格な評価、患者および職員のスクリーニング、消毒・滅菌方針の再検討にもかかわらず、アウトブレイクの制御は困難であった。このことは、当環境で十分に生残するとともに、長期的かつ無症状の保菌を引き起こし、間欠的な排菌を伴うという S. hadar の能力を示すものであると考えられる。
考察
初発症例は、疑わしい食品の摂取後に感染した母親であった可能性が高い。さらに、デジタル直腸体温計の潤滑用に黄色ワセリンのチューブを共用したために、感染が長期化したと考えられる。
結論
今回のアウトブレイクは、産科/新生児部門での S. hadar 感染のアウトブレイク制御は困難であること、また下痢症状を有するすべての患者を対象とした早期の便サンプル採取が重要であることを強く示している。
