The ‘My five moments for hand hygiene’ concept for the overcrowded setting in resource-limited healthcare systems

手指衛生は、医療関連感染症の予防を目的とした患者安全の中核的行動である。手指衛生実践を世界的に標準化するために、世界保健機関（WHO）は「医療現場における手指衛生ガイドライン（WHO Guidelines on Hand Hygiene in Health Care）」を公表するとともに、手指衛生が必要な場面を明確化するために、医療従事者の手指によって微生物が伝播することのエビデンスベースのモデルに基づいて「私の手指衛生の 5 つのタイミング」の概念を導入した。この概念の中心にあるのは、医療環境を患者区域（patient zone）と診療区域（healthcare zone）という 2 つの空間的なケア区域に分けるというものであり、医療従事者に特定の手指衛生機会の遵守を求めている。リソースが限られた過密な医療環境では、病床同士の間隔が不十分であるか、全くない場合が多い。このような条件下では、医療従事者には患者区域を視覚化・明確化する能力が求められる。「私の手指衛生の 5 つのタイミング」の概念は、共用されている患者区域の不明確さを最小限にするため、および WHO ガイドラインが設定した最終的な目標、すなわち感染症リスクの低下を達成するために、手指衛生の教育者、監査者、および医療従事者を支援することを目的として、このような環境に適用されている。

フィリピンやタイなどの発展途上国の一部の病院では患者数が非常に多く、1 つのベッドを 2 名の患者でシェアすることがある。WHO はこのような発展途上国においても可能な範囲で適切な感染対策が行われることを目標としている。本総説は D. Pittet ら WHO の「5 つのタイミング」の提唱者自身が、このような途上国における「5 つのタイミングの適用」について述べたものであり、「patient zone」を理解するのに、よい「頭の体操」になるだろう。
