Burden of gastroenteritis outbreaks: specific epidemiology in a cohort of institutions caring for dependent people

背景
要介護者のケアを行う施設ではウイルス性胃腸炎が高頻度に発生し、その伝染性は高い。高齢者では、これらが入院や、時には死亡をもたらすことがある。
目的
要介護者のケアを行う施設で胃腸炎アウトブレイクが及ぼす影響について調査すること。
方法
本研究は、18 施設の 4 領域（老年病・リハビリテーション科、老年精神医学科、高齢者ナーシングホーム、身体的・精神的障害を有する成人の専門的介護施設）の 35 ユニットで実施した。6 回の冬季中の胃腸炎アウトブレイクの時空間解析を実施し、施設の構造的特性（施設の規模および食堂の構成［個室または共用］）、ウイルスの疫学およびアウトブレイクの時系列、ならびに施設の領域について、臨床的データおよびウイルスのデータの解析を実施した。
結果
全 35 ユニットで合計 98 件のアウトブレイクを記録した。胃腸炎アウトブレイクのリスクは、国内の流行期以外でも高かった。86 件のアウトブレイクでウイルス検索を実施し、このうち 96.5％（86 件中 83 件）で同定された。内訳はノロウイルス遺伝子型 GII.4（59.0％、83 件中 49 件）、およびその他のウイルス（41.0％、83 件中 34 件）であった。胃腸炎アウトブレイクの頻度および発症率、ならびにウイルスの種類については、サーベイランスの時期によってばらつきがみられた。食堂の構成は、施設内の交差感染の要因であると考えられた。
結論
要介護者のケアを行う施設では、感染症の管理および医療従事者への情報を改善するために、正確な疫学に基づく特異的なサーベイランスを実施する必要がある。

想像を絶するウイルス性胃腸炎のアウトブレイクの実態報告である。集団生活を行う、いかなる施設においてもノロウイルスを始めとする感染性腸炎のリスクマネージメントは常に必要である。
