Reduction in Clostridium difficile environmental contamination by hospitalized patients treated with fidaxomicin

Fidaxomicin は殺芽胞作用を有しており、クロストリジウム・ディフィシル（Clostridium difficile）感染症（CDI）患者の治療に使用することによって、下痢の寛解までの時間が短縮すると考えられる。本研究では、すべての CDI 患者の治療に fidaxomicin を使用すると、C. difficile による環境汚染が減少するかどうかを調査した。メトロニダゾールおよび/またはバンコマイシンによる治療を受けた入院患者 66 例、ならびに fidaxomicin による治療を受けた入院患者 68 例の病室内の表面からサンプルを採取した。Fidaxomicin による治療を受けた患者（68 例中 25 例、36.8％）は、メトロニダゾールおよび/またはバンコマイシンによる治療を受けた患者（66 例中 38 例、57.6％）と比較して、患者の環境を汚染する割合が低かった（P ＝ 0.02）。Fidaxomicin による治療は、C. difficile による環境汚染の減少との関連が認められた。

メルク系列の子会社である Cubist Pharmaceuticals が欧米で発売しているクロストリジウム・ディフィシル感染症治療薬の fidaxomicin（フィダキソマイシン）は、欧米で実施された研究でクロストリジウム・ディフィシル感染症患者に対してバンコマイシンと同等かそれ以上の臨床治療効果が確認されている。国内では未導入であるが、今後の国内供給が待たれる。
