西アフリカにおける 2014 年のエボラアウトブレイクは主にギニア、シエラレオネ、およびリベリアで発生し、症例数および国際的対応は過去のあらゆるエボラアウトブレイクを超えるものであった。2014 年のアウトブレイク以前には、サハラ以南のアフリカでエボラウイルス病の大規模アウトブレイクは 20 件発生しており、2000 年のウガンダにおける 425 例、死亡率 53％が最大のものであった。1976 年のスーダンおよびザイールにおける最初のアウトブレイク以降、医療施設内での伝播は重大な懸念事項であり、医療従事者に感染をもたらし、市中への拡散増幅装置として作用してきた。院内伝播の主な原因は感染制御のためのリソースおよび個人防護具の不足である。地域の感染制御改善戦略に加えて、地域社会におけるこの疾患のとらえ方を十分に理解することが、アウトブレイク制御の促進をもたらしてきた。これまでのアウトブレイクから推奨されることは、疾患サーベイランスの改善による保健対応の迅速化、個人防護具の利用可能性の拡大、および国際的な備えの強化などである。

エボラ流行の惨事は、地域における文化・風習と医療機関における感染予防の知識と技術の遅れに加え、物資不足・人手不足という最悪のシナリオで進行した。今回の経験を今後の対策にどう生かすかが課題である。
