感染症やその他の疾患に関する個人レベルのリスク因子などの、医療データの電子的保管量は増大が続いている。これらのデータは、病院、地域、および国レベルで統合することが可能である。広範囲にわたる疾患のリスク因子や転帰などの情報を含むデータソースによって、様々な疾患の効率的な疫学的分析が可能となる。その影響は、医療プロセスのモニタリングにも及ぶであろう。多様なデータソースの統合には、疫学的、実務的、および倫理的な課題がある。例えば、診断基準、転帰の定義、および確認方法は、データソースによって異なっている可能性がある。データ量が膨大な場合があり、高度な情報処理技術が必要となる。大規模集団が対象の場合に最も大きな課題になると考えられる点は、統合データベースの作成のためのインフォームドコンセントをどのように取得すればよいかであり、特に、将来的に総合データベースを作成する可能性があることを容易に明示できない場合に当てはまる。本稿では、最近のプロジェクトにおけるいくつかの成功例・失敗例について、および抗菌薬耐性モニタリングのためのデータウェアハウジング^※の可能性について論じる。

活用までの速度、ボリューム、多様性、信頼性、社会的背景などの切り口で、データやその集積、さらに活用に必要な考え方を医療疫学に基づいて論じている。データが大きくなるにつれ、その活用にはより慎重になる必要があると考える。
