Association between students’ personality traits and hand hygiene compliance during objective standardized clinical examinations

背景
手指衛生の必要性は広く受け入れられているが、不適切な遵守を報告する研究が続いている。医学生は院内感染予防のための手指衛生の重要性について教えられており、正しい手指衛生手順の研修を受けている。しかし、性格特性（社会的志向および達成志向）が手指衛生遵守に影響を及ぼす可能性がある。社会的志向の高い人は、社会的な責任を感じ協力的に行動し、達成志向の高い人は野心的かつ競争心が強い。
目的
医学生の手指衛生遵守と性格特性との関連を評価すること。
方法
学生 155 名の手指衛生遵守状況を客観的臨床能力試験（OSCE）時に観察した。社会的志向および達成志向を、フライブルク性格検査改訂版（Freiburg Personality Inventory – Revised）の対応する尺度を用いて測定した。
結果
社会的志向は、手指衛生遵守率の高い学生と手指衛生遵守率の低い学生で相違がみられなかった（F₍₁₎ ＝ 3.87、P ＝ 0.052、η² ＝ 0.045）。達成志向には手指衛生遵守率の低い学生と高い学生との間に中等度の影響が認められ（F₍₁₎ ＝ 11.242、P ＝ 0.001、η² ＝ 0.119）、手指衛生遵守率の高い学生は、手指衛生遵守率の低い学生よりも達成志向が強いことが認められた。
結論
達成志向が OSCE 時に大きな役割を果たすのに対して、社会的志向は影響がそれほどみられない。著者の知る限り、これは学業達成場面において手指衛生遵守が達成志向と関連していることを示した初めての研究である。
