職業感染による急性 C 型肝炎と診断された医療従事者の治療成功★

医療従事者には C 型肝炎の職業感染リスクがある。英国では、2002 年から 2011 年に 17 例が職業感染による急性 C 型肝炎と診断された。17 例はいずれも中空針による経皮的損傷が原因であり、16 例では血液汚染が確認された。これらの医療従事者 17 例のうち、15 例は抗ウイルス療法を受け、14 例はウイルスの消失を確認することができた。治療成功は遺伝子型とは無関係であった。医療従事者の治療が成功したことから、職業感染による急性 C 型肝炎の管理に関する英国のガイドラインの必要性が示された。

監訳者コメント
針刺しによる経皮的な曝露後の HCV の感染発生率は約 2%といわれているが、職業感染による急性 C 型肝炎例について、その治療成績を示した報告は少ない。なお本報告では、曝露時とその後の血中 HCV RNA を測定し、陽転したものを急性肝炎例と定義している。C 型肝炎における近年の診断・治療上の進歩は著しく、それらをかんがみた曝露後の急性感染例のマネジメントに関する指針が望まれる。
