Ethanol versus heparin locks for the prevention of central venous catheter-associated bloodstream infections: a randomized trial in adult haematology patients with Hickman devices

血液内科の成人患者に対するエタノールロックによる中心静脈カテーテル（CVC）関連血流感染（CLABSI）予防効果は、十分に評価されていない。本研究の目的は、トンネル型 CVC 留置患者を対象として、ヘパリン加生理食塩液と 70％エタノールによる 2 時間のロックを前向きに比較することである。1,000 CVC 日あたりの CLABSI 発生率は生理食塩液群（43 例）6.0 件（95％信頼区間［CI］3.4 ～ 9.8）、エタノール群（42 例）4.1 件（95％CI 1.9 ～ 7.7）（P ＝ 0.42）であった。エタノール群では、出口部感染 2 件および皮下トンネル・ポケット感染 1 件が認められた。トンネル型 CVC 留置血液悪性腫瘍患者に対する 70％エタノールによる予防的ロックにより、デバイス関連感染は減少しなかった。

監訳者コメント：
CLABSI の予防および治療を目的とした様々な手法が研究されている。最近のシステマティックレビュー（Mermel LA, Alang N. J Antimicrob Chemother 2014;69:2611）では、エタノールロックによるカテーテルの品質劣化や患者の全身性の副作用なども示されている。本研究では少なくとも感染予防効果においてはヘパリンロックと比較して優位性も示せなかった。上記のシステマティックレビューの結果と併せて、CVC の CLABSI 予防としてのエタノールロックの有用性が否定されつつある。
