Impact of an educational intervention upon the hand hygiene compliance of children

背景
手指衛生遵守は、医療関連感染の減少を図るための単一の方法として最も効果的なものである。小児は感染症に罹患しやすく、また伝播の仲介役を担っている。著者らはこのような観察結果に基づいて、小児の手指衛生遵守の改善のための行動変化により、感染性疾患の伝播が減少するという仮説を立てた。
目的
小児の手指衛生遵守の強化とそれに伴う感染率の低下を目的とした長期的な行動変化がもたらされる教育的介入法を開発すること。
方法
双方向の教育セッション中に実施したフォーカスグループにより、小児の感性（what children felt）は、彼らの手指衛生遵守の強化に重要であることが確認された。このことを教育用器材の設計に反映し、次いでこの器材の手指衛生遵守改善効果の評価を行った。開発の第一段階は、5 ～ 8 歳の小児を研究参加者としてイーストミッドランズの 2 か所の学校で実施した。さらにこの器材を医療環境で使用し、設置場所に関する柔軟性の評価を行った。
結果
フォーカスグループから、小児は双方向の学習に興味を示し、感染症の交差伝播に関する知識が身につき、手指衛生遵守の改善を他者に促すようになることが示された。微生物のスワブ検査では、感染のリザーバとなり得る小児の手指および環境表面には病原体が存在することが確認され、質問票の調査では、介入後に手洗いが強化されたことが示された。
結論
教育的介入は、手指衛生を強化し、感染症の伝播を減少させる可能性がある。

監訳者コメント：
小児の場合、学年は同じでも情動行動や知的水準のばらつきが多く、それらを数値データとして反映するには無理があるかもしれない。より高学年の生徒や学生のほうが、評価対象に向いてる可能性が高い。
