Use of Xpert^® MRSA PCR point-of-care testing beyond the laboratory

メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）検査の所要時間短縮を図るために、整形外科の入院前外来および循環器部門で看護補助者が実施する迅速診断検査法（POCT）として PCR 法（Xpert^® MRSA、Cepheid）の評価を行った。POCT の結果を通常のスワブ採取の培養結果と比較した。POCT は操作が容易であり、陰性結果が得られるまでの所要時間は著しく短縮した。感度は入院前外来で 75.0％、循環器部門で 80.0％であった。POCT 陽性・培養陰性が 27 件にみられたが、これらの患者には MRSA 感染・保菌は処置後 1 年以上認められなかった。臨床検査室を介さない MRSA 保菌の POCT 検査には、臨床検査室内で実施する方法を上回る重要な利点があり、さらなる検討を進めるべきである。

監訳者コメント：
病棟で MRSA の迅速診断を行うことの有用性を検討した論文である。PCR 法による結果と培養結果が一致したのは 97.5％（30 件）であった。結果が出るまでに要した時間は、PCR 法で約 1 時間、培養では41時間（結果陰性）から139時間（結果陽性）であった。これらの結果は、病棟での迅速診断が治療や対策に十分有用であることを示唆している。
日本では、MRSA 感染が発生すれば患者 1 人あたり 5,212 円/日のコストがかかるという報告もあり（Kobayashi H, et al. 環境感染誌 2010;25(2):111-112）、入院時からの対応には迅速診断が役立つと思われるが、PCR 法による費用も気にかかる。特に保菌の場合の対策については、入院時のリスクアセスメント結果との比較に興味がもたれる。
