Outbreak of meningitis due to Serratia marcescens after spinal anaesthesia

本稿では、帝王切開時に脊椎麻酔を受けた患者のセラチア・マルセセンス（Serratia marcescens）による髄膜炎のアウトブレイクについて報告する。75 床の私立病院で 2011 年 3 月 6 日から 14 日に脊椎麻酔下で帝王切開を受けた患者 46 例中 12 例が、細菌性髄膜炎と診断された。ノルアドレナリン含有 5％ブドウ糖液のプレフィルドシリンジ 4 本および輸液バッグ 1 個から採取したサンプルから S. marcescens が分離されたことから、脊椎麻酔に使用した薬剤の外因性汚染が示唆された。手術室での麻酔に関連した感染の予防戦略について考察する。

監訳者コメント：
Attack rateは26％、1 日あたりの attack rate は一番高い日で 67％（9 例中 6 例）であった。注射バイアルの複数回使用、調整後の 14 日間にわたる室温保管など、明らかに薬剤調製にかかわるアウトブレイクであった。発症者 12 例が全員回復したのが救いである。
