New screening method to detect carriage of carbapenemase-producing Enterobacteriaceae in patients within 24 hours

カルバペネマーゼ産生腸内細菌科（CPE）保菌患者を迅速に特定することは、病院内での CPE の侵入および伝播の予防に必須である。本稿では、入院後 24 時間以内に CPE 保菌患者を検出するための新規スクリーニング法の成績について報告する。高リスク患者の直腸および咽頭スワブを採取して一晩培養し、DNA 分離後にリアルタイム ligation-mediated PCR 法を用いて保有率が高い CPE 遺伝子（KPC、NDM、OXA-48、VIM、IMP）の検査を行った。14 か月間で 454 例のスクリーニングを実施し、6 例から CPE が検出された（保菌率 1.3％）。

監訳者コメント：
本検討は、「search and destroy」と形容されるような非常に厳格な感染予防策を実施し、国外の病院に入院歴のある患者すべてについて保菌スクリーニングを行っているオランダで行われた。CRE 検出患者は南欧やアジア、アフリカで入院歴があり、KPC、OXA-48 陽性であった。国外からの輸入の実態・頻度を明らかにし得たのは、国策として上記の体制を取っているオランダゆえのことかもしれない。迅速かつ確実な伝播予防につながったと予想するが、今後、検査でカバーできる微生物の範疇、検出能の改良、費用といった課題も有するであろう。前項のアイルランドの論文と比較して読むと、CRE の状況と対策の違いをより明確に把握できる。
